The provider confirms the address, equipment capability, timing, pricing, and terms. Ask what assessment is proposed and what approval is required before work proceeds.
What you provide
- The facility name, city, street address, and the best dock or door identifier.
- Whether the equipment is a leveler, sectional door, rolling door, high-speed door, restraint, seal, shelter, bumper, or a fire-rated assembly.
- What happened, when it was first noticed, the current position of the equipment, and how many positions are affected.
- Whether a trailer is at the position and whether the facility has restricted access or operating windows.
- A make or model only if it is available in existing records or on a safely accessible label. "Not sure" is useful and acceptable.
- The name or role of the person authorized to discuss scope and approve work.
Keep clear of visibly damaged or unexpectedly moving equipment, stop using the affected position, and follow the facility's established safety procedures. For immediate danger to people or an active fire, use emergency services and facility emergency procedures rather than this request line.
What the provider confirms
- Whether the facility address is within its service area.
- Whether it works on the equipment type, make, configuration, and applicable fire-rated assembly.
- Its available timing and how access or shutdown needs affect scheduling.
- How assessment, service-call charges, estimates, authorization, and pricing work.
- Who would perform the work and the terms, documentation, warranty, and payment requirements it offers.

Who contracts for and performs the work
The facility decides whether to accept a provider's proposal or authorize work. The provider is responsible for describing its scope, personnel, charges, terms, and work. Questions about an assessment, workmanship, parts, warranty, payment, or invoice should go to the provider identified on its proposal, work order, or invoice.
Routing a request does not transfer the facility owner's or employer's legal responsibilities. The facility remains responsible for its people, procedures, equipment controls, vendor approval, and decisions about workplace operations. Appropriate authorities and qualified advisers determine requirements for a particular site or project.
